The form
One question at a time.
Jordan answers on their phone, in a calm form that asks one thing at a time and needs no Salesforce license of their own.

Baseline to six months
Progress, including the slow months.
Each follow-up is compared with Jordan's baseline. Housing held flat for two assessments before it moved, and a caseworker's note records what changed.

The whole journey
Every form Jordan fills out, on one path.
Intake, needs, housing, check-ins, and feedback stay linked to one person, so the slow stretches and the progress both show.

When your funder asks
Ready when your funder asks.
Each of these stories becomes a report a funder can read: one participant's progress, one program's outcomes, or every program at once. A number on its own invites questions. In these reports, the answers sit beside it.
- Out of how many?
29 of 39
participants measured are improving in Housing Stability.
- Compared to when?
Each person's first assessment. 35 have been measured twice so far, and the 4 measured once aren't counted as not improving.
- Could that be chance?
Confirmed means the change is backed by a minimum sample and statistical checks, so it's unlikely to be noise.
- And the smallest groups?
In a report, any group with fewer than 5 participants shows as <5, with no exact count or percent.

SLCM Neighbor Network
Before OutcomeLens, intake meant staff filling out forms about a neighbor, and the most important parts of their journey lived in scattered spreadsheets. Today OutcomeLens enables client-led intake: neighbors tell us who they are and what they need, in their own words. That information flows straight onto one record with their assessments and care plan, so our navigators start every conversation already informed and nobody falls through the cracks. It has changed both how we work and how neighbors experience us.
Clare Wallace, Executive Director
Louisville Urban League
Louisville Urban League has worked with Clint Majors for the better part of a decade. He helped us learn Salesforce from the ground up, led our move to the Nonprofit Success Pack, and became the person we turn to on nearly every data decision. When he started building OutcomeLens, we wanted to help shape it.
As a Founding Partner, we put our real programs in front of it: Jobs, Education, Housing and financial empowerment, Black Business, Expungement, Health, and Scholarship. Our feedback is in the product, and our new welcome form and program intakes are built on it.
What I value most is that OutcomeLens lives inside the Salesforce we already use. Nothing new to log into, nothing to export. The work our staff do every day becomes the outcomes story we tell our funders and the National Urban League.
